ABSTRACT

The impact of the COVID-19 pandemic and related restrictions on alcohol consumption in Australia remains unclear. High-resolution daily samples from a wastewater treatment plant (WWTP) which served one of the largest cities in Australia, Melbourne, were analysed for temporal trends in alcohol consumption under extended periods of COVID-19 restrictions in 2020. Melbourne experienced two major lockdowns in 2020, which divided the year of 2020 into five periods (pre-lockdown, first lockdown, between lockdown, second lockdown and post second-lockdown). In this study, daily sampling identified shifts in alcohol consumption during different periods of restrictions. Alcohol consumption in the first lockdown period, when bars closed and social and sports events ceased, was lower than pre-lockdown period. However, alcohol consumption was higher in the second lockdown period than the previous lockdown period. There were spikes in alcohol consumption at the start and end of each lockdown (except for post lockdown). For most of 2020, the usual weekday-weekend variations in alcohol consumption were less evident but there was a significant difference in alcohol consumption between weekdays and weekends after the second lockdown. This suggests that drinking patterns eventually returned to normal after the end of the second lockdown. This study demonstrates the usefulness of high-resolution wastewater sampling in evaluating the effects on alcohol consumption of social interventions in specific temporal locations.

ABSTRACT

BACKGROUND: Australian harm reduction services are provided via a mix of modalities, including fixed-site needle and syringe programmes (NSP) and syringe-dispensing machines (SDMs). SDMs are cost-effective and provide 24-h anonymous access to needles/syringes, often to underserved geographic areas, and can attract clientele who may choose not to use NSPs. The introduction of COVID-19 control measures saw disruptions and adaptations to the provision of harm reduction services. It is possible that SDMs filled the gap in otherwise disrupted harm reduction services in Melbourne. In this paper, we use data from four SDMs and an NSP to explore changes to harm reduction usage during periods of COVID-19 lockdowns in Melbourne, Australia, in 2020. METHODS: Our data span September 2017-December 2020. We analysed daily counts of SDM use and monthly counts of NSP use, according to unique presentations to both. Auto-regressive integrated moving average (ARIMA) time-series models were fitted to the data with the effects of lockdowns estimated via a step function. RESULTS: Across the study period, we estimated 85,851 SDM presentations and 29,051 NSP presentations. Usage across both the SDMs and the NSP declined during the COVID-19 lockdowns, but only the decline in SDM usage was significant in ARIMA analysis. CONCLUSIONS: The slight, but significant decline in SDM use suggests barriers to access, though this may have been mitigated by SDM users acquiring needles/syringes from other sources. The decline, however, may be a concern if it led to lowered needle/syringe coverage and a subsequent increase in injecting risk. Further work is needed to properly explore potential changes in preference for needle/syringe acquisition site and associated barriers. Importantly, this work adds to the body of literature around the impacts of COVID-19 on harm reduction provision and potential areas of improvement.

ABSTRACT

OBJECTIVE: The aim of this study is to examine the relationship between alcohol consumption, psychological distress and COVID-19 related circumstances (being in lockdown, working from home, providing home-schooling and being furloughed) over the first eight months of the pandemic in Australia. METHOD: A longitudinal study with six survey waves over eight months with a convenience sample of 770 participants. Participants were aged 18 or over, lived in Australia and consumed alcohol at least monthly. Demographic data was obtained in the first wave. Data on alcohol consumption, psychological distress (Kessler 10), and COVID-19 related circumstances (being in lockdown, working from home, providing home-schooling and being furloughed) were obtained in each survey wave. RESULTS: Results from the fixed-effect bivariate regression analyses show that participants reported greater alcohol consumption when they had high psychological distress compared to when they had low psychological distress. Meanwhile, participants reported greater alcohol consumption when they worked from home compared to when they did not work from home. Participants also reported greater alcohol consumption when they provided home-schooling compared with when they did not provide home-schooling. The fixed-effect panel multivariable regression analyses indicated a longitudinal relationship between higher psychological distress and providing home-schooling on increased alcohol consumption. CONCLUSION: Broader drinking trends during the COVID-19 pandemic typically indicate increases and decreases in drinking among different members of the population. This study demonstrates that in Australia, it was those who experienced psychological distress and specific impacts of COVID-19 restrictions that were more likely to increase their drinking.

ABSTRACT

Aims Restrictions to alcohol availability during the first and second waves of the COVID-19 pandemic in Australia included closures of businesses where alcohol is sold for on-premises consumption (pubs, bars, restaurants and cafes), but not where alcohol is sold for off-premises consumption (take-away and home delivery). This study aimed to compare beer consumption before and after restrictions to alcohol availability during the first and second waves of COVID-19 in Australia. Design Interrupted time-series analysis. Setting Australia. Participants Estimated resident population aged 15+ years. Measurements Seasonally adjusted estimates of beer per capita consumption measured in litres of alcohol (LALs) per week, disaggregated by on- and off-premises sales. Findings First-wave restrictions (week beginning 23 March 2020) were associated with a significant immediate reduction in on-premises beer per capita consumption [-0.013 LALs, 95% confidence interval (CI) = -0.013 to -0.012, P = < 0.001], but no significant change in off-premises beer per capita consumption. Partial removal of first-wave restrictions (week beginning 18 May 2020) was associated with a significant immediate increase in on-premises beer per capita consumption (+0.003 LALs, 95% CI = 0.001 to 0.004, P = 0.006), but no significant change in off-premises beer per capita consumption. Second-wave restrictions (week beginning 06 July 2020) were associated with a significant immediate reduction in on-premises beer per capita consumption (-0.004 LALs, 95% CI = -0.006 to -0.002, P = 0.001) but, again, no significant change in off-premises beer per capita consumption. Conclusion Restricting the availability of on-premises alcohol during the first and second waves of the COVID-19 pandemic in Australia was associated with significant reductions in on-premises beer consumption, but no significant changes in off-premises beer consumption. ABSTRACT

INTRODUCTION AND AIMS: Restrictions introduced to reduce the spread of COVID-19 have had major impacts on the living circumstances of Australians. This paper aims to provide insight into shifts in alcohol consumption and associated factors during the epidemic. DESIGN AND METHODS: A cross-sectional convenience sample of 2307 Australians aged 18 and over who drank at least monthly was recruited through social media. Respondents were asked about their alcohol consumption and purchasing in 2019 prior to the epidemic plus similar questions about their experiences in the month prior to being surveyed between 29 April and 16 May 2020. RESULTS: Reports of average consumption before (3.53 drinks per day [3.36, 3.71 95% confidence interval]) and during (3.52 [3.34, 3.69]) the pandemic were stable. However, young men and those who drank more outside the home in 2019 reported decreased consumption during the pandemic, and people with high levels of stress and those who bulk-bought alcohol when restrictions were announced reported an increase in consumption relative to those who did not. DISCUSSION AND CONCLUSIONS: A reported increase in consumption among those experiencing more stress suggests that some people may have been drinking to cope during the epidemic. Conversely, the reported decrease in consumption among those who drank more outside of their home in 2019 suggests that closing all on-trade sales did not result in complete substitution of on-premise drinking with home drinking in this group. Monitoring of relevant subgroups to assess long-term changes in consumption in the aftermath of the epidemic is recommended.
